Sclera and Atrophic Lichen – General Information

Sclero-atrophic lichen is a chronic inflammatory nature of the anogenital area affecting predominantly women, but can also affect men and can have extra-genital locations. It is considered a disease of adults but can occur in the age group 1-13 years. Women are affected 10 times more frequently than men.
The etiology of the disease is unknown, but no studies proving a connection between this and infection with Borrelia.
Sclero-atrophic lichen is present with genital and extragenital manifestations. Skin lesions are represented by polygonal papules pearly-white, ivory or porcelain shade, which can be merged together. Pilosebacee holes or dilated sweat glands may be obstructed by plugs of keratin, making the skin look rough and thickened. The lesions may accompany the bubble, erosions, purpura.
Hyperkeratotic areas on the vulva can erode the skin becoming macerated. Vulvar region becomes atrophic, with reduced clitoris size. Lesions may be pruritic or painful if there is erosion, especially during walking and may be associated with dysuria (pain on urination) and dyspareunia (pain during intercourse).
The initial lesions appear uncircumcised men the foreskin, which in turn becomes the sclera and can be no withdrawn, leading to phimosis. The glans appears white, ivory-colored skin, thin, semi-transparent, and small hemorrhages may surface in the form of crimson. Balance of the appellant is a frequent complication.
Treatment
Disease management is extremely important because of the potential devastating hypoplasia of the clitoris and labia minora.
Local treatment:
- Clobetasol propionate have proven effective for genital lichen sclero-atrophic, but their use should be limited to no more than 6-8 weeks
- Pimecrolimus and tacrolimus topical corticosteroid similar benefits
- Topical androgens are rarely used now because of the risk of clitoral hypertrophy
Systemic treatment:
- Hydroxychloroquine 125-150 mg / day, a few weeks-months, to monitor potential side effects
- In men the symptoms of phimosis and circumcision in some cases can cause disease remission.
